Entries should be made using proper upper and lower case, including proper legal names as listed on legal documents.  Do not use all capital letters or nicknames.

Before you login to begin the process please review and obtain the following required documents which will need to be uploaded in the Document Upload section.

• Residency Verification Affidavit (download from the Documents section)

• 2 Proofs of Residency (current mortgage, lease, utility bill with name & address on it)

• Verification of Parent/Guardian Identity (drivers license, passport, government-issued ID)

• Proof of Age (birth certificate, passport, alternate certificate)

• Proof of Immunizations

• Health Exam for CA School Entry (only for new TK-1st grade, and first-time students to CA; download from the Documents section)

• Oral Health Assessment Form (only for new K-1st grade students; download from the Documents section)

• Proof of Withdrawal from previous school, if applicable (Gr 1-12)

• Academic Report Card or Unofficial Transcript (Gr 1-12)

• Record of Prior Special Programs (download from the Documents section)

• Restraining Order (if applicable)
